When the Nebraska Health Care
Funding Act was passed in May 2001, 16 new multi-county health departments were created.
As a result, all Nebraska counties are now covered by a local health department. The
Office of Community Health Development provides technical assistance to all local public
health departments and monitors the funds that are distributed under the Act.
New Funding Opportunity
This Request for Proposals (RFP) seeks proposals from local public health departments
that have completed the Mobilizing for Action through Planning and Partnerships (MAPP)
process; have written an intervention plan to address one of the following areas and/or
the risk factors related to them: cardiovascular disease, diabetes, cancer, injury, and/or
tobacco; and have not previously been funded under this RFP. Through this RFP, the
Nebraska Office of Community Health Development (NOCHD) will award up to $180,000 in grant
funds to local health departments in Nebraska. Proposals are due May 15, 2008.

Facilitated by public health leadership, this tool helps communities prioritize public
health issues and identify resources for addressing them.
