You Will Need:
Your Social Security number for a profession
MyLicense number (found on your renewal notice) for a facility
Mastercard or Visa (Debit or Credit)MILITARY: Nurses can not request this fee waiver: If you have served in the regular Armed Forces of the United States, during part of the twenty-four (24) months immediately preceding the biennial renewal date, and wish to waive the renewal fee, you must submit documentation of your military orders BEFORE PROCEEDING. You may scan and email to rita.thalken@dhhs.ne.gov, or fax (402) 471-3577, or mail to Licensure Unit, PO Box 94986, Lincoln, NE 68509-4986. Please be sure to include your name, profession, license number, and email address in the fax, email, or written correspondence.
